Description: This project is intended to be the reception assessment facility, which will provide a high level of support that is aimed at stabilising and engaging with individuals who have mental health problems and related behaviours. This project will provide approximately 6 hours of support per client per week and aims to build a solid foundation for engagement with services. It is envisaged this project will provide the transition to settled independent living. The support offered is visiting in nature and will consist of individual key working and organised house meetings. The support is aimed at maintaining current levels of need and developing skills to maximise independence and plan for a structured move to less supported housing. Clients will have access to out of hours support and management for emergencies. Our underlying philosophy is one of providing access for socially excluded vulnerable adults through structure, planning, information and support. 

Accessibility: Ground Floor bedrooms
Opening Times: 9-9, 7days per week
Referral Routes: Care Trust, Health, Housing Department, Non-Statutory agencies, Probation, Self-referral, Social Services Department
Exclusions: No specific exclusions – Decided on a case by case basis

Description: This project will be the second stage of move-on for clients leaving stage one. The scheme is designed to consolidate and further improve life skills and ensure continued engagement with both clients’ individual support plan and any external agencies that they may be involved with. The support offered is visiting in nature and will consist of individual key working and organised house meetings. The support is aimed at maintaining and improving levels of need. Support staff will work with the client to maximise independence and plan for a structured move to less supported or independent accommodation. Clients will have access to out of hours support and management in event of emergencies.
